Ondo Air Crash: Investigation Shifts Focus To Military Aviation Safety.

The crash of a Nigerian Air Force aircraft in Ondo State has renewed questions about safety standards in military aviation, with calls for investigators to look beyond the immediate cause and examine the wider systems governing aircraft maintenance, operations and record-keeping.

The ATR-42 aircraft, identified as NAF 931, crashed near Igbokoda in the Ilaje Local Government Area of Ondo State on Monday, October 5, while on a routine mission from Benin to Lagos. The Air Force later confirmed that 25 people — 20 passengers and five crew members — were on board, with no survivors.

Former Director of the Department of State Services, Mike Ejiofor, says the circumstances surrounding the incident warrant a broader review of military flight operations. Speaking on Wednesday, he questioned the reported composition of the aircraft and urged authorities to examine its maintenance history, age and operational condition as part of the investigation.

The former security chief also raised concern over the initial discrepancy in the number of people aboard the aircraft. While preliminary information from the Ministry of Aviation indicated 32 occupants, the Nigerian Air Force subsequently reviewed its manifest and confirmed 25. The Air Force has since released the names of the five crew members and 20 passengers who died.

Ejiofor said establishing the cause of the crash will depend heavily on recovering the aircraft’s black boxes and other evidence from the site. The wreckage came down in a difficult, swampy area near a naval base, complicating the recovery operation being conducted by the Navy, Marine Police and other responders.

He also cautioned against drawing conclusions from reports of a loud sound heard before the aircraft went down. According to him, the noise could have resulted from the impact, although investigators would have to determine whether there was any explosion before the crash. No official finding has so far established either explanation as the cause.

The Federal Government has ordered an investigation into the crash, while the Chief of the Air Staff has directed a comprehensive inquiry. The Air Force has said the purpose is to establish what happened and strengthen safety measures across military aviation operations.

With the investigation now underway, attention is shifting from the immediate recovery effort to the lessons that can be drawn from the tragedy. Beyond identifying the direct cause, a full review of maintenance, flight procedures, passenger records, emergency response and operational oversight could help determine whether there are wider safety gaps requiring correction.
